SCAN Foundation Issues RFP to Expand GRACE Team Care in California - Aug. 7, 2012

The SCAN Foundation – which works to improve care for seniors – has issued a Request for Proposals (RFPs) to support the replication of Geriatric Resources for Assessment and Care of Elders (GRACE) Team Care in California. GRACE Team Care, which was first developed at Wishard Health Services in Indianapolis, is a cost-effective team care model that has been proven to improve the health and reduce avoidable readmissions for frail, lower income, older adults. The GRACE team works with patients in their homes and their community to manage health problems, track changing care needs and leverage social services. Grantees will receive training and technical assistance to implement GRACE Team Care in their organization. Selected organizations will also receive reimbursement for travel for onsite training in Indianapolis. The RFP is open until Sept. 4.
