Comprehensive Primary Care Initiative

The Comprehensive Primary Care (CPC) initiative is a new CMS-led, multi-payer initiative fostering collaboration between public and private health care payers to strengthen primary care for all Americans. The CPC initiative will test two models simultaneously: a service delivery model and a payment model. The service delivery model will test comprehensive primary care.

Next Steps:

Once these markets agree to the terms and conditions of the initiative, the Innovation Center will release the application for primary care practices to participate in the chosen markets. Approximately 75 practices will be chosen for each market and will be enrolled once they agree to offer the expanded package of services called for by the initiative.
