Special Populations

The US health care system understands that “special needs” populations may have additional needs in terms of health care, from transportation to supervision to help maintaining independence. Individuals in need of additional assistance or care may include those who have disabilities, the aging, have chronic medical conditions, the young, are homeless or live in institutionalized settings. To address these and other special populations, NAPH members are employing innovative programs that aim to boost outcomes, as well as contain costs.
