Camino de Salud Network Provider Practice

In 2007, LAC+USC partnered with a non-profit health care corporation to improve access to specialty care. Jointly, they developed and implemented the provider practice redesign model, the Camino de Salud Network Provider Practice, which has improved access to specialty care by shifting the paradigm of how private clinic primary care providers (PCPs) and public county hospital specialists in the LAC+USC Medical Center service area interact and care for patients.

The redesign work centers on improving access to the “right care at the right time”, improving appropriateness of referrals from primary care community clinics to specialty care through the development of jointly developed specialty care/diagnosis guidelines and enhanced primary care training, and improving access to and efficient use of diagnostic services through decentralization of these services and application of developed guidelines.
