Good Health Program

The goal of the Good Health Program is to provide outreach to vulnerable Contra Costa Health Plan members, in ethnically and financially disparate populations, who inconsistently attend medically necessary health assessment appointments or routine check-ups with their primary care providers. Historically, a challenge for this population is ensuring continuity of care between multiple providers.

Interventions range from appointment assistance and transportation coordination to translation services and case management to improve health outcomes. The goal is provide personalized information and support regarding all aspects of accessing health care services, including financial barriers and concerns, mental health services, medical equipment and devices, and medication reconciliation.

Patients with special needs – frail elderly, the disabled, and those with multiple chronic illnesses – frequently need additional assistance and care coordination. A comprehensive medical and psychosocial assessment is completed for each patient to identify risks and quickly intervene to keep members in their homes and maintain the patients’ quality of life.

The implementation of the Good Health Program has provided Contra Costa with evidence supporting the effectiveness of “high touch” programs, telephone outreach, and using multi-disciplinary teams to maximize personnel resources. The program has also enhanced communication across all levels of care.
