Health care reform efforts have focused on curbing escalating health care costs through the reform of delivery systems and payment changes. Studies have shown that, when properly implemented, health information technology (HIT) would save money and improve the quality of health care. As the Centers for Medicare & Medicaid Services continues to implement the Medicaid and Medicare electronic health record incentive programs, NAPH members must determine how their hospitals will meet the criteria for Meaningful Use.

Participants joined NAPH and the team from Healthcare Information and Management Systems Society Analytics (HIMSS) as they discussed:

1. How NAPH members compare with hospitals across the country using the HIMSS Analytics EMR Adoption Model,

Palliative care does not only provide care to the dying, but can offer many individuals with chronic pain or chronic disease alternative interventions and therapies to reduce or eliminate symptoms. This specialty also provides emotional support to both the patients and families as they make difficult decisions during difficult times. Palliative care became a recognized specialty in 2006. What is only now coming into focus is that teams of experts can improve the quality of the care experience and decrease costs at the same time. Dr. Solomon Liao and Dr. Christine Ritchie discussed the value of palliative care to improving the patient and family experience during difficult times while reducing the cost of care for a vulnerable patient population.

9/13/11: A Discussion on a Proposal to Create Safety Net Accountable Care Organizations - Conference Call

At the end of last year, NAPH met with Dr. Rick Gilfillan who heads the Center for Medicare and Medicaid Innovation (CMMI). He encouraged us to submit a proposal for a safety net accountable care organization (ACO) project centered on our members. Late last year, we received feedback from you on the framework for such an ACO project. At this point, we have developed a more detailed proposal and wanted to get your feedback before we submitted this more detailed proposal to the CMMI.

We discussed the following questions on the call:

1. How should criteria be set so that as many NAPH members who choose to can participate in a Safety Net ACO demonstration project?

2. How should patient attribution be accomplished?

3. Which Medicaid population should be included (e.g., fee-for-service (FFS) only or FFS and managed care)?

4. Which Medicaid payments should be at risk for shared savings (e.g., base payments only or base and supplemental payments)?

5. Which risk models are you ready to implement (e.g., FFS with shared savings, partial capitation, full capitation, shared savings only, or shared losses as well)?

6. How much risk are you willing to take on (e.g., are the suggested savings rates of 60-80% adequate)?

7. How would a Safety Net ACO project work in states with Medicaid managed care?

8. How can ACOs incorporate uninsured patients into a Safety Net ACO project?

These were just some of the questions that we wanted to discuss. NAPH plans to submit a proposal to Dr. Gilfillan in the next couple of weeks. Thank you for joining us on the call.

The Affordable Care Act will expand coverage for low income patients with complex needs. However, it also prescribes reductions in funding sources that are used to support hospitals that care for a disproportionate share of Medicaid and uninsured patients. Going forward safety net financing policy should support and ensure funding for core safety net missions. As new safety financing policies are developed, it is important to understand the current sources of funding for unreimbursed care.

4/28/11: Improving Emergency Department Throughput: Innovations at Safety Net Hospitals - Webinar

Emergency department (EDs) overcrowding is on the rise-48% of EDs within the U.S. are at or over capacity, compared with 65% of urban hospital EDs and 73% of teaching hospital EDs. The challenges of managing patient overflows are especially acute in safety net hospitals. Insurance coverage expansion beginning in 2014 could lead to a greater shortage of primary care doctors, and thus more demand for safety net hospital ED services. Despite these obstacles, NAPH members have achieved extraordinary results navigating the most vulnerable patient populations through overwhelmed EDs. 4/26/11: Detailed Analysis of Proposed Medicare ACO Program - Webinar

The Centers for Medicare & Medicaid Services released its long-awaited proposed rule on the Medicare Shared Savings Program ("MSSP"), establishing Medicare Accountable Care Organizations ("ACOs"). This represents another step in a long line of efforts to shift Medicare from a quantity-driven to a value-driven purchaser of health care services. The proposed MSSP would reward ACOs for delivering integrated care at lower costs while meeting quality standards.

4/6/11: The New Accountability Measures with The Joint Commission's Dr. Jerod Loeb

The article, Accountability Measures-Using Measurement to Promote Quality Improvement, written by Drs. Chassin, Loeb, Schmaltz and Wachter describes a framework for evaluating measures based on their ability to improve the care delivered to individuals by maximizing health benefits to patients.

Dr. Jerod Loeb describes the model and how the Joint Commission will be applying this framework in their activities of the future. In addition, Dr. Loeb gives his perspectives on how the framework should be extended beyond the Joint Commission into the work of the National Quality Forum as they develop measures moving in to the future.

2/9/11: New Medicare Hospital Value-Based Purchasing Program - Webinar

The Centers for Medicare & Medicaid Services (CMS) released a proposed rule to implement a Hospital Value-Based Purchasing (VBP) program, as required by the Affordable Care Act. As statutorily mandated, Medicare payments would be reduced beginning in fiscal year 2013 to fund incentive payments to hospitals achieving certain quality-based performance scores. Members gave feedback on the impact of the proposed rule. Member comments will guide NAPH's submission of comments. A brief summary of the content of the proposed rule can be accessed here.

On this conference call, NAPH receives member feedback on a proposal to the Center for Medicare and Medicaid Innovations (CMMI) for safety net accountable care organizations. Members discussed the potential criteria for participation in the ACO, the type and amount of investments that hospitals need to launch an ACO, necessary infrastructure for launching, metrics ACOs should meet, and the interaction between an ACO project and existing waivers and how supplemental payments are handled.

Dr. Jim O'Connell discusses the Boston Healthcare for the Homeless Program. Dr. O'Connell and his team have developed a program to meet the needs of this vulnerable population using the principals of public health. The program involves a comprehensive electronic medical record, tracking longitudinal data on key metrics, compliance for certain screening interventions for preventive care, and outcome measures such as mortality. The program's Board of Directors is composed of individuals who are currently or have been homeless. Later, Becky Kanis from Common Ground discusses the national 100,000 Homes Campaign, developed in conjunction with the IHI to provide sustainable housing for 100,000 of the most vulnerable chronically homeless individuals. Individuals are prioritized as most in need of housing by the Vulnerability Index - developed by the Boston Healthcare for the Homeless team. Finally, Elizabeth Boyce from LA County Department of Health Services discusses the current activities supported in her community to develop appropriate and comprehensive services for this group of individuals. 9/30/10: Building Accountable Care Organizations (ACOs): A Conversation with Carilion Clinic's Dr. Edward Murphy - Webinar

Since the passage of the Affordable Care Act, there has been significant buzz about the creation of accountable care organizations (ACOs), or a group of provider organizations that are accountable for cost and quality of patient care. Pioneering work on ACOs is underway given the efforts of the Brookings-Dartmouth ACO Learning Network, several private insurers, and even individual states. These efforts will inform guidance issued by the U.S. Department of Health & Human Services on its Medicare Shared Savings program this fall.

Dr. Edward Murphy is viewed as one of the nation's pioneers in creating an ACO. On this webinar, Dr. Murphy described the challenges, best practices and lessons learned in creating an accountable care organization. Carilion Clinic in Roanoke, Virginia, is an integrated delivery system that includes eight hospitals and 525 employed physicians (as well as 75 affiliated physicians), undergraduate and graduate medical education, and home care operations. Carilion converted from a hospital-centric health care organization to a multispecialty, patient-centered clinic in 2006. Like many safety net health systems, Carilion's drive to accountable care included a focus on: Improved population management; Increased physician leadership and accountability for quality, patient satisfaction, and efficiency of operations; and Increased primary care capacity.

In August, the Office of Consumer Information and Insurance Oversight (OCIIO) published a notice seeking comments to aid OCIIO in the development of standards for establishing and operating state health insurance exchanges. As the health reform landscape shifts, safety net health systems recognize that effective economic relationships with commercial health plans are key. Many safety net health systems patients will qualify for subsidized health insurance via state exchanges and qualified health plans. Additionally, intense pressure on Medicaid and Medicare cost containment will continue post reform given the federal fiscal deficit and strained state budgets.

On this conference call, Noah Rosenberg who has worked on behalf of physician groups and hospital systems with commercial health plans for over 25 years, addressed the changing health plan market and implications for safety net providers.

This presentation focused on the Agency for Healthcare Research and Quality (AHRQ), Health's MacColl Institute, RAND Health, and the California Health Care Safety Net Institute's (SNI) newly developed toolkit that safety net organizations may use to implement The Chronic Care Model. This spearkers described the toolkit and the "Safety Net Institute and Public Hospital Chronic Care and Business Strategies Pilot Project" and lessons learned. The National Quality Forum (NQF) is a not-for-profit membership organization created to develop and implement a national strategy for health care quality measurement and reporting. The purpose of this call was to provide an overview of NQF's national goals and priorities. Dr. Burstin, Senior Vice President for Performance Measures at NQF, described how NQF processes measures for development, review, and endorsement of quality and patient safety measures. She also described NQF's health care quality national priorities, and the role NAPH members can play on expert panels and technical advisory committees. Speakers from member hospitals discussed their experiences serving on NQF technical advisory committees and how they were able to influence national measures.
