HITS Program

The HITS Program (Health Intervention with Targeted Services Program) at Memorial Healthcare System in Hollywood, FL, provides effective and cost-efficient solutions to uninsured and underinsured patients, while reducing uncompensated care costs for taxpayers and the health care system. The program improves the health status of the community by reaching out to uninsured and underinsured patients who are in need of primary care—linking these patients to a medical home to reduce avoidable hospital and emergency room visits. During the last two years, the HITS Program has touched the lives of more than 4,000 persons in the South Broward County community and linked 725 persons to a medical home. It has connected 225 persons to Medicaid providers and more than 500 persons to Memorial’s Primary Care Program, which offers primary and preventive services at three conveniently located centers. The goal of this program is to ensure that 100% of program participants are assessed for health insurance eligibility and understand the importance of establishing a medical home. The HITS Program was recently awarded the Sapphire Award by the Blue Foundation for a Healthy Florida, the philanthropic affiliate of Blue Cross and Blue Shield of Florida (BCBSF).
