The Wraparound Project
Many violently injured youth and young adults are not provided “reentry” opportunities upon hospital discharge in a manner similar to the criminal justice system. Physical rehabilitation is provided in the aftermath of injury, however, providing services to reduce or eliminate risk factors associated with violent injury are not traditionally offered upon hospital discharge. The overall purpose of the Wraparound Project is to reduce interpersonal violent injury and incarceration rates by providing comprehensive risk reduction services. In addition, the purpose is to institute a model for sustained partnership between the acute care trauma center and the affected communities in order to provide a more complete approach to the care of violently injured individuals.
The Wraparound Project provides mentorship and links clients to essential risk-reduction resources in order to reduce injury recidivism and prison recidivism in the most vulnerable citizens in San Francisco. The Project aims to:
- Develop a new paradigm for a trauma system’s care of interpersonal violent injury victims to minimize risk of future injury, or recidivism.
- Institute culturally sensitive case management from the affected communities during the acute care of this population to facilitate improved health communication and assist individuals in linking to regional and community risk reduction resources.
- Decrease activities and behaviors associated with violent injury with comprehensive rehabilitation, linking acute care to community and municipal risk reduction resources.
- Adapt public health evaluation measures historically applied to unintentional injury and substance abuse to a violence prevention protocol.
- Decrease modifiable risks, evaluated as intermediate indicators associated with interpersonal violent injury, using comprehensive rehabilitation with culturally sensitive case management and links to resources.
Since the inception of the program, 435 people have been eligible for the Project, and it has been lauded as an innovative program that has revolutionized long-term rehabilitation for violently injured youth and young adults.
