CMS to Hold Meeting on Transition Care Services Pilot Program - November 11, 2010

The federal Centers for Medicare & Medicaid Services (CMS) will hold a meeting on Dec. 3 in Baltimore, Md., to discuss the Community-Based Care Transitions Program. The program was conceived under the Affordable Care Act and is intended to develop evidence-based transition care services that reduce hospital readmissions. Prospective payment system hospitals with high readmission rates may be eligible for participation in the pilot program, which will run 2011-2016. Each hospital must implement at least one evidence-based transition care service and may collaborate with community-based organizations. To attend the meeting in person, participants must register by Nov. 19; those planning on joining the webcast must register by Dec. 2. Information and registration details are available online.
