HHS Launches Initiative of Multiple Chronic Conditions - December 21, 2010

The U.S. Department of Health and Human Services (HHS) on Dec. 14 issued a report titled, “Multiple Chronic Conditions: A Strategic Framework.” The report outlines the strategic vision of HHS agencies on how to address the unique needs facing the more than one in four Americans who suffer from multiple concurrent chronic conditions. Among other objectives, the report advocates for using health information technology, facilitating home and community-based services, enhancing health professionals training, and addressing disparities among varied populations.
