CMS Announces Web Conference on Community-Based Care Transitions Program - May 3, 2011

The federal Centers for Medicare & Medicaid Services (CMS) recently announced it will hold a national Forum on May 5, from 1:00 PM to 2:30 PM Eastern to educate individuals on the CMS Community-Based Care Transitions (CBCT) Program. The Forum will help answer questions about the CBCT Program and provide an opportunity for listeners to ask questions. The CBCT Program, authorized under the Affordable Care Act, will provide funding to hospitals and community-based health care settings in order to build links between providers, help transition individuals from inpatient settings to other care settings, improve care for Medicare beneficiaries, and reduce hospital readmission rates.
